How to apply VOR and VOT ?
1) Verification of Registration (VOR) – Apply through LJM (KKM) by hardcopy courier post
For VOR, please fill up this form VOR Application Form and prepare bank draft RM30, attach together with your photocopy nursing registration certificate (malay & english). And post it to Malaysia Nursing Board.
LJM (Malaysia Nursing Board) will send your registration directly to Singapore nursing board.
Apply VOR for all of your nursing registration such as: Community Nurse, Assistant Nurse, Registered Nurse, Midwife (prepare bank draft follow the form, each registration cost RM30 bank draft)
We will need you to constantly contact LJM (03-88833561) for update once you have post out to them.
SUMMARY to prepare for VOR Application
- Photocopy LJM cert (Malay) x 1 copy
- Photocopy LJM Cert that has been translated into English and CTC x 1 copy
- Bank Draft RM30 for 1 verification, under the name of: Secretary of the Malaysia Nursing Board – To be done in the bank, just tell them you want to do Bank Draft
- Fill up VOR Application Form OR Fill up online & Download at https://www.digisigner.com/online/showTemplate?linkId=9024847d-3bee-4af8-a126-7cffb17e1914
Put all documents above into envelop that bought in Post Office, Courier post all documents to:

LJM will take 6-8 weeks to get this done.
2) Verification of Transcript (VOT) – Apply through College by email/call (If your school has been closed down, read SECTION 2)
For VOT, you have to approach your college directly and get them to send your transcript with full details of grades and reflect the detailed breakdown of all theory subjects in theory hours/weeks and clinical practice hours/weeks directly to Singapore Nursing Board through email softcopy, when email softcopy, please ensure college do not cc you inside the email. but please BCC [email protected] (send recruitment agent a copy)
Apply VOT for all of your nursing qualification such as: Community Nurse, Diploma, Post Basic, Degree

After college send the VOT to SNB email address, please request for them to forward you the email and please screenshot the email (include the date and time college email to SNB) WhatsApp to your recruitment agent when you received.
**VOR and VOT are to be sent**
PREFERABLY by email to [email protected] via the relevant authority’s official email. The sender needs to include full name and designation and documents must be sent via the institution’s official email account. (Non-official email accounts such as Gmail, Yahoo mail, etc. will not be accepted.)
OR by mail to Registrar, Singapore Nursing Board c/o Secretariat of Healthcare Professional Boards (SPB) (81 Kim Keat Road #10-00 Singapore 328836) and to provide us with the date sent and tracking no. (if by mail) once available.

SECTION 2: What to do if College has been closed down?
If college has been closed down, unable to get them to send VOT
Please apply both VOR and VOT through LJM:
1. Fill up both VOR Application Form and VOT Application Form
2. Go to bank prepare bank draft RM60 (in the name of Secretary of the Malaysia Nursing Board) , amount follow as stated on both forms. (eg: total amount is RM60, can make RM60 in 1 bank draft)
3. Put all in envelope : attach required documents (as stated on the application forms) such as LJM (Malay and English with CTC) x 2 copies, Transcript with full details with CTC x 1 copy, Bank Draft and VOR & VOT application form
4. Using Courier service such as post laju, courier post to LJM

SECTION 3: After All VOR and VOT application DONE
- VOR will take 6-8 weeks for LJM to email directly to SNB (started from receipt date), you will receive 2 emails.
- First Email: LJM will email you the Receipt of your bank draft payment (usually around 1-2 weeks after courier post, if never receive, probably your documents may lost somewhere)
- Second Email: To inform you date and time they email to SNB (Please screentshot this email, WhatsApp to your recruitment agent when you receive this, as example below)
